Fill in the Word document form as detailed as possible. When you are done, please file, 'save as' a PDF document and change the file name from '"Project Questionnaire" to your "First/Last Name". Once you have converted the file to a 'PDF' from a '.docx' send it to me as an email attachment to: firstname.lastname@example.org
Please title the subject line as: "COMPLETED PROJECT QUESTIONNAIRE" in all capital letters. This will allow me to organize my emails and find your questionnaire faster.
Write a brief email letting me know that you have completed the questionnaire and it is ready for me to review. You may also add any additional information that may not have been on the questionnaire.
If it is easier for you, you can print the form out, fill it in by hand, scan it and send it to my email. Use the same instructions specified above.
